Initial Applications

Who to Contact

First-time applicants who are Burlington residents can apply through one of the following offices:

  • For Town of Burlington Residents (under 55 years): Contact Youth and Family Services at 781-270-1961.
  • For Town of Burlington Seniors (55 years and older): Contact the Council on Aging at 781-270-1950.
  • For United States Veterans of the Armed Forces (regardless of age): Contact Veterans Services at 781-270-1959.

Assistance with the Fuel Assistance application is available from November through April on a first-come, first-served basis.


Appointment Only

Assistance with the application is available by appointment only.


Cancellations

If you need to cancel your appointment, please inform our office at least 24 hours in advance. Failing to cancel or communicate will require you to visit the fuel assistance office in Lowell to complete your application.


Required Forms/Information

When applying, please provide the following documents:

  • Heating Bill: A copy of your current heating bill.
  • Identification: Photo ID for the head of household and ID for all household members.
  • Income: Current gross income from all sources for all household members (last 30 days). This includes:
    • Weekly Pay: 4 consecutive pay stubs if paid weekly, or 2 if paid bi-weekly.
    • Over 18 Students: A letter from the school if children 18+ are full-time students.
    • No Income: A signed no-income statement (provided at the office) if over 18 and claiming no income. If claiming no income for over a year, provide a wage statement from the Social Security office.
  • Rental Subsidy: Current tenant profile if you have a rental subsidy.
  • Fixed Income: A copy of your most recent check or printout if receiving fixed income (Social Security, Disability, SSI, etc.).
  • Pension: A check stub, letter from pension company, or 1099 document showing the current gross amount.
  • Child Support: Copies of four current payments or a court order with a current check or stub, or a notarized statement from the ex-spouse indicating the amount and frequency of payments.
  • Workers Compensation: Four most recent check stubs.
  • Unemployment: Award status document.
  • Interest Income: Documentation of interest income of over $100.
  • Self-Employment: A signed copy of the prior year’s entire tax return, including attachments. If self-prepared, the signature page must be notarized. A transcript will also be requested.
  • Multi-Family Homeowners: If you own a multi-family home, provide:
    • Current signed copy of your entire Federal Tax returns, including Schedule.
    • Homeowners insurance.
    • Mortgage interest statement.
    • Property tax bill.
    • Rent receipts from tenants.
    • Water/Sewer bill.
